Not for emergencies. Call 911 if experiencing medical emergency.

Nature of Telehealth

Telehealth involves the remote delivery of health services using electronic communications including video, audio, messaging, and the electronic exchange of medical images and records. You will interact with a licensed clinician who will conduct a Good Faith Exam to evaluate your suitability for requested therapies.

Risks & Limitations

As with any healthcare delivery model, telehealth carries inherent limitations. Technology failures, transmission delays, or incomplete information may affect the quality of the interaction. In some cases, an in-person examination may be necessary before a clinician can prescribe. You should ensure you have a stable internet connection and a private, well-lit environment for your visit.

Alternatives

You have the right to seek in-person care from a locally licensed clinician at any time. Telehealth is not a replacement for emergency care or acute medical services. If you are experiencing a medical emergency, call 911 immediately.

Good Faith Exam Requirement

A Good Faith Exam will be conducted by a licensed clinician as required by applicable state law before any prescription is issued. This exam evaluates your health history, stated goals, and relevant risk factors. Payment does not guarantee that a prescription will be issued following the exam.

Privacy & HIPAA

Your personal health information will be protected in accordance with HIPAA and applicable state privacy laws. Your information will not be sold or disclosed to third parties except as required for treatment, payment, or healthcare operations. See our HIPAA Notice of Privacy Practices for complete details.

Off-Label Use & Compounding

Your clinician may discuss off-label uses of approved medications as part of clinical judgment. If prescribed, compounded medications are prepared by a state-licensed 503A compounding pharmacy. Compounded drugs are not FDA-approved and have not been reviewed by the FDA for safety or efficacy. Your clinician will discuss risks, benefits, and alternatives with you.

State Licensure & Jurisdiction

By proceeding, you confirm that you are physically located in a state where our services are available and that the clinician conducting your visit is licensed to practice in that state. Services are not available in Alabama or Mississippi.

Your Consent

By completing the intake process, you acknowledge that you have read and understood this Telehealth Consent, and you agree to receive telehealth services under these terms. You retain the right to withdraw consent at any time by contacting us at admin@newbluehealth.com.
