This Notice describes how Protected Health Information (PHI) about you may be used and disclosed by New Blue Health and its affiliated medical group, and how you can access this information. Please review it carefully.
Permitted Uses & Disclosures
We may use and disclose your PHI without written authorization for the following purposes: providing treatment and coordinating care, processing payment and billing, conducting healthcare operations (quality assessment, training, licensing), public health activities, regulatory oversight and law enforcement as required by law, research with appropriate safeguards, and to avert a serious threat to health or safety.
Uses Requiring Your Authorization
We will not use or disclose your PHI for marketing purposes or sell your PHI without your written authorization. Most other uses and disclosures not described in this Notice will require your prior written authorization. You may revoke any authorization in writing at any time, except to the extent we have already acted on it.
Your Rights Regarding Your PHI
You have the right to: inspect and obtain a copy of your PHI; request that we amend your PHI if you believe it is incorrect or incomplete; receive an accounting of disclosures we have made; request restrictions on how we use or disclose your PHI; request that we communicate with you by alternative means or locations; and receive a paper copy of this Notice upon request.
Our Duties
We are required by law to maintain the privacy of your PHI, provide you with this Notice of Privacy Practices, notify you following a breach of unsecured PHI, and abide by the terms of the Notice currently in effect. We reserve the right to change our privacy practices and the terms of this Notice.
Filing a Complaint
If you believe your privacy rights have been violated, you may file a complaint with us at admin@newbluehealth.com or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
